A printable weekly diary built around the 12 PIP activities and the reliability rules - so every tick you make is evidence a decision maker has to weigh, not just a note.
PIP is not decided on your diagnosis. It is decided on how your condition affects 12 everyday activities on the majority of days - and whether you can do each one safely, to an acceptable standard, repeatedly and in a reasonable time. The problem: by the time the form or the assessment arrives, nobody remembers three Tuesdays ago. A diary kept day by day does.
What is inside
A full month of daily pagesFour weeks, 28 daily pages - all 12 PIP activities with tick columns that map to the legal tests: struggled, took much longer, needed help or prompting, unsafe or incident, could not do it.
Weekly + monthly summariesEach week counts your bad days against the "more than half" rule; the monthly summary totals the month (15 or more of 28 = your typical days) and captures your three worst incidents - your strongest evidence.
How-to guideTwo minutes each evening: record the worst moment of the day, not the average, and never downplay. Written the way assessors actually ask.
Review protectionDay-to-day fluctuation is the normal pattern of your condition, not a change. A running diary proves it before any review or renewal.

The diary gathers the evidence. The points are scored on the PIP2 form. See how one of your own activities reads as an answer written the way decision makers score it, free, no card, about two minutes.
Most symptom trackers ask how you feel. This one asks what a PIP decision maker must know: for each activity, could you do it reliably today? Each column corresponds to one of the four reliability tests in the PIP rules, and the weekly count lines up with the "majority of days" principle. When you later write your form, prepare for an assessment or face a review, you mine the diary for real, dated examples - "on Tuesday I could not stand long enough to cook and went without a hot meal" beats "I struggle with cooking" every time.
The diary gathers the evidence. Our service turns it into the wording decision makers score against - personalised answers for all 12 activities based on your conditions.

Why does a diary help a PIP claim?+
PIP is decided on how your condition affects 12 everyday activities on the majority of days, tested against the reliability rules: safely, to an acceptable standard, repeatedly and in a reasonable time. A diary kept day by day is contemporaneous evidence of that pattern - far stronger than trying to remember your worst days when the form or the assessment arrives. Every tick box in this diary maps to one of those legal tests.
Does the diary work for a PIP review or ADP review?+
Yes - reviews are where it shines. Day-to-day fluctuation is the normal pattern of a fluctuating condition; reviews ask about longer-term change. A diary kept in the months before your review shows that your ups and downs are the same condition behaving as it always has, not an improvement.
Is my information safe?+
We only ask for your email address to send you the diary and occasional PIP guidance. The diary itself is a printable PDF - everything you write in it stays on your paper, with you. We never see it.
