PIP for Health Anxiety 2026: Descriptors and How to Claim

Can you get PIP for health anxiety? Yes. Health anxiety (illness anxiety disorder, sometimes still called hypochondria) is assessed on how it affects your day to day life, not on the diagnosis label. When a persistent, overwhelming fear of being seriously ill - with constant checking, reassurance-seeking and avoidance - takes over your life, it can score points across several PIP activities, most often Activity 9 (engaging with people), Activity 3 (managing therapy) and Activity 10 (making budgeting decisions).

Health Anxiety Is More Than Worrying About Your Health

Health anxiety is a recognised mental health condition. It involves a persistent preoccupation with the fear of having, or developing, a serious illness, which does not settle even after medical reassurance or normal test results. It often drives time-consuming behaviours: repeatedly checking your body, searching symptoms online, seeking reassurance, and frequent GP or hospital contact - or, for some people, the opposite, avoiding anything medical because it is too frightening. The DWP does not score the worry itself, but it does score the functional limits that severe health anxiety creates, so your job on the form is to translate the condition into those limits.

The Reliability Test: Your Most Important Protection

Under regulation 4(2A) of the Social Security (Personal Independence Payment) Regulations 2013, you only count as being able to do an activity if you can do it:

The case law in RJ, GMcL and CS v SSWP [2017] UKUT 105 (AAC) confirmed that "safely" includes the risk of psychological harm, not only physical harm.

The "4-Point Rule" Was Scrapped: What It Means for You

In 2025 the government proposed requiring at least 4 points in a single daily living activity to qualify for the daily living component (Clause 5 of the Universal Credit and Personal Independence Payment Bill). After a backbench rebellion, Clause 5 was removed on 1 July 2025 and dropped permanently at the bill's third reading on 9 July 2025. There is no new single-activity threshold. Which PIP Activities Does Health Anxiety Affect?

Health anxiety is mostly a daily living condition. Activity 9, Activity 3 and Activity 10 are the usual scorers, with Activity 11 where avoidance affects journeys.

When health fears dominate, engaging with people becomes hard: you may avoid others, dominate conversations with health worries, or be too distressed to take part. Descriptor (c) at 4 points applies where you need a trusted person to manage engagement (appointments, calls, social contact); descriptor (d) at 8 points where attempted engagement triggers overwhelming distress so you cannot do it at all on the majority of days. Activity 3: Managing Therapy or Monitoring a Health Condition

Activity 3: Managing Therapy (daily living)

  1. Does not receive medication, therapy or supervision 0
  2. Needs no more than once-a-week supervision, prompting or assistance 1
  3. Needs more than once a week but no more than 3.5 hours a week 2
  4. Needs more than 3.5 hours but no more than 7 hours a week 4

Add up the weekly time on treatment and management: CBT or counselling, medication, exposure and response-prevention homework, and the time someone spends helping you resist checking or supporting you through medical contact. Where a partner or family member has to spend hours each week managing your reassurance-seeking and appointments, descriptor (c) or (d) (2 to 4 points) can be reached.

Severe anxiety and rumination can impair concentration and decision making. If someone else has to manage finances or prompt you through decisions because anxiety makes you freeze or spend impulsively (for example on private tests, supplements or appointments), descriptor (b) or (c) applies for 2 to 4 points.

Activity 11: Planning and Following Journeys

If health anxiety means you avoid going out (fearing illness, contamination, or being far from help), or you can only travel to medical settings, Activity 11 descriptors can apply. Where you need prompting to undertake journeys to avoid overwhelming distress, descriptor (b) gives 4 points; where you cannot travel without another person, descriptors (d) to (f) give 10 to 12 points. How much is YOUR PIP worth?

Describe Your Worst Days, Not Your Best

PIP is scored on whether you can do an activity on more than half the days. People with health anxiety often lose points by describing a calm day. Instead, describe how often the fear takes over, how much time goes on checking and reassurance, what you avoid, and the cost of getting through.

I worry a lot about my health. I check my symptoms and go to the doctor more than most. I cope at home.
Strong (4–8 points)
On most days I am consumed by the fear that I have a serious illness. I cannot manage appointments or phone calls without my partner, because I become so distressed I cannot follow what is said. I spend 2 to 3 hours a day checking my body and searching symptoms, and my partner has to manage our money and decisions because I cannot. I have cancelled work and social events repeatedly. After a bad episode I am unable to function for the rest of the day.

The strong description scores because it shows frequency, who must help, the time lost to checking, the effect on decisions, and the recovery cost. That points towards Activity 9, 3 and 10.

Key phrase for your form: "On the majority of days I cannot engage with people or make decisions without support because overwhelming health-related fear takes over. Hours each day are lost to checking and reassurance. Per regulation 4(2A) I cannot do these activities safely, to an acceptable standard or repeatedly."

The Assessment Is the Hardest Part

A face-to-face assessment can itself trigger severe anxiety, and attending one is evidence of how disabling the condition is. You can request a phone, video or home assessment as a reasonable adjustment under the Equality Act 2010, and ask for a trusted person to be present. If you do attend, make sure the assessor records how it affected you: were you accompanied, did you struggle to speak, did you need reassurance, did you take extra medication beforehand?

Do not minimise it. If you spend hours a day checking, cannot make decisions, avoid the phone, or rely on someone to get you through the day, say so plainly. Playing it down on the day is the most common reason genuine claims are refused.

Treatment and Medication

Health anxiety is usually treated with CBT, often including exposure and response prevention to reduce checking and reassurance-seeking, sometimes alongside medication. NICE lists SSRIs such as sertraline as a treatment option for anxiety. Treatment does not always remove the condition, and relapse is common, so PIP is scored on your current function, not on the fact that you are being treated. Record any medication side effects, because they can affect daily activities too.

If You Are Refused

Mental health claims are refused more often than they should be, frequently because an assessor underestimates severity. Do not give up - a large share of refusals are overturned later.

  1. Mandatory Reconsideration (within one month of the decision letter). Ask for the assessment report, then dispute each activity, quoting regulation 4(2A) and citing RJ v SSWP [2017] UKUT 105 for "safely" including psychological harm. See our full Mandatory Reconsideration guide.
  2. First-tier Tribunal (within one month of the MR result). Around two in three tribunal appeals succeed. You can usually attend by phone or video, which is far easier with severe anxiety, and bringing a representative roughly doubles the success rate.

Can you get PIP for health anxiety?

Yes. Health anxiety (illness anxiety disorder, sometimes called hypochondria) is assessed on how it affects you day to day, not on the label. Can you claim PIP for health anxiety if you can still work or leave the house sometimes?

Yes. PIP looks at whether you can do each activity reliably - safely, to an acceptable standard, repeatedly and in a reasonable time, on the majority of days. Forcing yourself through tasks while consumed by health fears, or needing hours of checking and reassurance to manage, is not doing them reliably. Many people who manage on the surface still qualify because of the hidden cost.

How much PIP can you get for health anxiety?

It depends on your scores. PIP has a daily living part (standard £76.70/week, enhanced £114.60) and a mobility part (standard £30.30, enhanced £80.00) for 2026/27. Severe health anxiety that affects engagement, decisions and the ability to go out can reach both components. The maximum, if you get both enhanced rates, is around £10,119 a year.

Which PIP activities does health anxiety affect most?

Activity 9 (engaging with people) is common, where worry and avoidance make being around others overwhelming. Activity 3 (managing therapy) can apply because of the time spent on checking, GP and hospital contact and treatment. Activity 10 (making budgeting decisions) can apply when rumination and anxiety impair decisions. Activity 11 (planning and following journeys) applies if you cannot travel without overwhelming distress.

Is health anxiety the same as hypochondria for PIP?

For PIP it does not matter what it is called. Health anxiety, illness anxiety disorder and the older term hypochondria all describe the same kind of overwhelming, persistent fear of illness. PIP is scored on the functional impact, not the diagnosis label, so describe what the condition stops you doing and provide evidence of diagnosis and treatment.

What evidence helps a PIP claim for health anxiety?

Letters from your GP, psychiatrist or psychology team confirming the diagnosis and severity; any NHS Talking Therapies referral or discharge letters; a current medication list; and a signed statement from someone who knows you describing what you avoid and what they do for you. A short diary of checking behaviours, cancelled plans and the days you could not function is persuasive because it is concrete.

My health anxiety comes with depression or OCD. Should I claim for both?

Yes. List every condition that affects you, because PIP looks at the combined functional impact, not a single label. Health anxiety very commonly occurs with depression and overlaps with OCD-style checking. Claiming all the conditions that affect you gives a fuller, more accurate picture and usually scores higher than one alone.
